Suddenly in need of care: your plan in six questions
After a fall, a stroke or an operation, a lot has to be arranged at home at once. This tool asks six questions and turns your answers into a plan: what to do before discharge, what matters on the day itself, what comes in the first week and what follows the decision of the care insurance fund (Pflegekasse).
What the plan covers
- Applying for a care level (Pflegegrad), step by step, with a finished application as PDF
- Finding a home care service: all 723 services from the open data of the State of Berlin, sorted by distance from your postcode
- Choosing aids (Hilfsmittel) and knowing who prescribes and who pays
- Medication, the discharge letter and the first visit to the family doctor
- Short-term care (Kurzzeitpflege) if going home is not yet possible
- A home emergency call for people who live alone
The operator of this site appears at the top of the list if it covers your postcode. You choose freely who provides care.
